Liposuction surgery also known as liposculpture, liposplasty and suction-assisted lipectomy is the most frequently performed cosmetic surgery procedure done in the United States. It is best performed on those that are physically fit, but are still unable to get rid of fatty deposits in there stomach, thigh or hips.

Previously liposuction cosmetic surgery required blood transfusions due to the amount of blood loss during the procedure. This all changed when Dr. Jeffrey Klein, a dermatologic surgeon created the tumescent technique which allows a patient to go under local anesthesia instead of general anesthesia minimizing blood loss.

Dermatologic surgeons performing tumescent liposuction surgery showed that complications have been minimal attributing only to anesthesia or to large volume liposuction. The liposuction procedure, abdominoplasty or abdominal liposuction has been linked with fatalities. The American Society of Liposuction Surgery studied over 15,000 patients with no deaths reported. Other symptoms that were reported were; toxic shock, skin irregularities, hematoma, unacceptable scarring, sensory nerve impairment and dermatitis. Another current survey showed out of 66,000+ liposuction procedures, no deaths were reported.

The tumescent liposuction procedure not only uses local anesthetic but a small microcannulae. Using a small cannulae to form the tunnels in the tissue allows for a more effective less traumatic fat removal thus causing less pain and minimal blood loss. With less pain for patients lowers the amount of anesthetic required during the liposuction procedure.

As with any cosmetic surgery procedure patients must have realistic expectations in considering the liposuction procedures outcome. If the patient has cellulite it may improve but not expected to lessen. Other than in good physical health, they should be on a well-balanced diet to aid in healing and maintain the results of the liposuction plastic surgery. Crash dieting before the procedure could increase risks of electrolyte imbalance or nutritional deficiencies. All the patients’ motives for having the procedure should be recognized during the initial consultation. Expecting situations to improve in their life such as their marriage or employment are not reasons to have cosmetic surgery, of any kind.

The patient needs to disclose their medical history in full detail. This should include; all medication both prescription and over the counter, supplements, medication allergies, prior surgeries, medical diseases, family medical history. An outline of the all the risks need to be discussed and understood along with instructions for post surgical care. Most cosmetic surgeries require a week or more of recovery and longer before you can resume your regular daytime activities. Also the surgeon should provide you with a liposuction without surgery or any other alternative procedure, if available. The insertion marks will be planned and placed in hidden areas while the patient is wearing clothing, such as a bikini. Then the before liposuction surgery pictures will be taken for comparison with the after photos taken at a later date post surgery.

During the consultation the client should also ask questions of the surgeon. If he/she is certified with the American Board of Plastic Surgery and how many liposuction procedures have they performed. Also ask to see other patient’s liposuction surgery photos or patient referrals. What are the liposuction surgery costs and if they have a pay plan or if they use a health care credit company. Many health care credit companies will allow guaranteed financing for patients with bad credit at a lower interest rate than standard credit card companies. They also have higher credit limits, some as high as $25,000.

Common areas for liposuction include upper and lower abdomen, hips and flanks (love handles), inner and outer thighs, inner knees, arms, neck, ankles, calves and back. Recently liposuction has been used for the traditional breast reduction, but leave inverted-T scars.

During the liposuction procedure the skin will be prepared with a disinfectant and the insertion sites are marked. A local anesthetic is used to permeate the skin of the insertion sites. The sites are then cut, widened and pretunneled using a small hemostat. The anesthetic is delivered to the deepest layer and some surgeons add corticosteroid to decrease postoperative soreness and infection. The liposuction cannulae are inserted and the surgeons “brain hand” guides the surgeon to the areas that need to be liposuction and determines the endpoint of surgery. At this time the surgeon can have the patient wake up and give his/her opinion regarding any missed areas and immediately return to the table to complete the surgery. Doing this reduces the number of secondary procedures.

After surgery some surgeons will suture the sites or let them heal with second intent to allow for more drainage, less bruising, and less inflammation. Compression garments and absorptive pads are applied for the immediate postoperative period and worn days to weeks depending on the surgeon. Swelling after the surgery will decrease within the first two weeks. As the swelling goes down you will need to get a smaller garment. Remember to follow the surgeon post surgical instructions on personal care (bathing etc.), restrictions for daily activities and any follow-up appointments. Liposuction is performed on an outpatient basis, requiring only several hours, and the patient can return to home that evening. Normal activity level can occur within a few days to a couple of weeks. This all depends on how many areas the patient had liposuction or if other procedures were performed.

Recently a new liposuction method has gained attention because it requires less downtime than other traditional liposuction procedures. The laser liposuction or SmartLipo (smart lipo) uses a pulsed system to help heat (laser) that actually shatters fat cells, and the resulting oily, liquid substance is then removed through a tiny incision in the skin by the surgeon. The small laser can also seal blood vessels as it zaps fat, so there is less swelling, bleeding and bruising than with traditional liposuction. Research has shown that little, if any, fat is reabsorbed back into the body. The minimal invasiveness of the procedure allows patients to see faster results and to experience fewer complications. SmartLipo is also known as the “lunchtime lipo” although it is suggested to take a day or two off work many patients only need a small area liposuction and with the aid of a compression garment and some over-the-counter pain medication go right back to work.

Another method, LipoSelection is an ultrasonic liposuction procedure, meaning it uses ultrasonic energy as a means of fat removal. The ultrasound technology breaks up and softens the fat that is to be removed. Unlike other fat removal methods, LipoSelection leaves the surrounding nerves, blood vessels, and tissue virtually unharmed. After a local anesthetic is applied, the surgeon will fill the area to be treated with a saline wetting solution to numb the site and shrink blood vessels. This helps minimize post-surgical bruising and swelling. A small probe emitting high frequency sound energy is used to break up fat on contact with minimal damage to surrounding tissues. Fat may then be easily extracted with liposuction tubes.
